On 10 July 14 Care Navigators completed the 9 month training programme, they were trained in relevant skills, including motivational interviewing, difficult conversations, record keeping. They heard from 26 different organisations and services in Croydon. This enables them to help patients access the best care.
Dr Yusuf Rajbee presented the certificates. During the final celebrations we heard how some of the CNs have used the training.
I work with a couple who now ask for me. The wife has dementia. I regularly sort out their medication, arrange GP appts and talk about social needs and the husband’s carer needs.
I work with a young patient with cancer. He can’t read or write. I signposted to SE Cancer Centre and Purley Cross for support and help with money and told them to come and see me any time.
Patient lost his home when he fell behind on his payments after an injury. He ended up on the streets. Thankfully I was able to signpost him to Crisis to help make difference in his life.
